School Registration

Dear Prospective Parents,
Registration 2009 - 2010 are still being accepted: Registionationis $330.00 per child and Tuition for 1 child is $360.00 a month.

Preschool registration is $175.00 per child 3 & 4 year old and must be fully potty trained. 1/2 day tuition Monday through Friday is $350.00 a month (7:30 am to 3:00pm) or full day Monday through Friday is $550.00 a month (7:00 am to 6:00 pm)

All of the reistration materials for the 2009-2010 school year are included in this link. If you have any questions while completing this form please feel free to call the school's office at (909) 822-4431. The non-refundable registration fee is due at the time of registration. the registration process is not complete until all forms are turned in and completed.

The following forms are to be turned in at the time of registration for your students:

A) Tuition Contract
B) Physical Exam, and current immunization record for grades K-1, K-2, 1st, 4th, and 7th grades. (These are all due before the first day of school)
C) Registration Form
D) Copy of birth certificate
E) Copy of last report card
F) Baptismal certificate (if you have one)
G) Emergency Card
H) Automatic Tuition Deduction Form
I) Fingerprint form
J) Safety Environment Certificate
K) Record Request Form

Reminder: Please keep collecting Box TOP, Campbel Labels and ink cartridres, as Resurrection earns points for turning these in.

Resurrection is participating in the Fresh and Easy Shop for Schools Program. Please save your receipts and bring them into the school office. All receipts must be dated September 16th through December 31st.
